Please know we will respond to refill requests within 72 hours of receiving your request.

Refills for controlled substances do require an office visit. 

Please make sure you are up to date on your office visit to ensure refills are appropriate. 

Appointment Cancellation: 

Please allow at least 24 hour notice to avoid a $40.00 no show fee.

To schedule your appointment we will need to know what you are being seen for in order to schedule the correct amount of time you will need to spend with our provider.  Hope Clinic currently accepts uninsured or underinsured, Medicare, Medicaid and certain commercial carrier insurance patients.


Forms & Documents Needed for Your First Visit

Before coming to the Hope Clinic for your first appointment, please print out and complete the necessary new patient forms.

Also, bring a copy of one of the following documents to verify your service charge rate:

  • 2 consecutive months pay stub
  • Most recent W-2 form
  • Statement from employer verifying income
  • Shelter letter
  • Treatment program letter
  • Letter from case worker
  • Letter from church or other health center

Complete Your Lab Panel

In order to make the most of a Chronic Care patient's initial visit to the Hope Clinic and prevent subsequent visits where a patient will incur additional costs, a panel of lab tests, known as the Hope Panel, is performed on a new patient prior to their first appointment. The Hope Panel is a blood test that is comprehensive for lipid disease, cardiac biomarkers, liver panel, and a CBC (complete blood count). The Hope Panel is $75 for uninsured patients. Outside of the Hope Panel, other laboratory tests including blood work and urinalysis are provided at a price to the patient that is 60 percent less than traditional laboratory tests.

A licensed practitioner will interpret and review with patients the final results of all lab work done at our facility. To obtain the lab results, patients must make a follow-up appointment with one of the Hope Clinic providers.

What if I cannot make it to my scheduled appointment?

To avoid a late cancelation or no show charge it is advised to call to cancel or reschedule at least 24 hours before your expected appointment time.

Does the Hope Clinic allow walk-ins?

Pending provider availability, walk-in office visits are available and limited to existing Hope Clinic patients that are sick. Walk-in hours are as follows: 

Monday, Tuesday, and Thursday:  7:30 am- 11:30 am

Wednesday and Friday: 7:30 am- 11:30 am

Does the Hope Clinic help with prescriptions?

The Hope Clinic works with Pharmaceutical Companies that offer prescription assistance to the uninsured, as well as being knowledgeable of prescription discount pricing within the community. Our goal is to help each patient find solutions to cut down the out of pocket cost to each patient.

What if I need care or testing beyond what the Hope Clinic offers?

The Hope Clinic has relationships with over 100 specialist within our community that treat our uninsured at little or no cost.

The Hope Clinic has arrangements within the community with local partners to receive deeply discounted services for further testing such as CT Scans, MRI's
